Please fill this out for EACH of your facilities. It should take no more than 30 seconds.

Please do this for both SEEK and Stedi- the 2 insurance verifications portals we use to run the checks.**

What to do:

1. Go to: https://www.cms.gov/data-research/cms-information-technology/hipaa-eligibility-transaction-system-hets/hets-edi-how-enroll

2. Log into your MAC portal (same one used for claims)

3. Complete the HETS EDI Enrollment form

What you'll need:

— Your NPI (Individual or Group — whichever you are enrolled with Medicare with)

— Your PTAN

— Authorized signer's name

— Your email address

— Relationship effective date: Today’s date (or any prior date)

— SEEK's Clearinghouse ID: E9C5

— Stedi’s Clearinghouse ID: 274B
